Lumbar Facet Syndrome: Symptoms, Diagnosis and Options
Lumbar facet syndrome describes pain that may arise from the small joints at the back of the vertebrae. Its pattern overlaps with other causes of low back pain, so symptoms, examination findings and imaging must be interpreted cautiously.
Pain that is more noticeable with extension or rotation, located near the spine and sometimes referred to the buttock or upper thigh, may suggest a facet component. No single movement, manual test or MRI finding confirms the source by itself.

How can a facet hypothesis be recognized without jumping to conclusions?
Pain is often axial and paraspinal, with greater sensitivity during prolonged standing, back extension or rotation. It can refer to the buttock, groin or thigh without following a precise nerve pattern. Pain below the knee, weakness or numbness calls for assessment of radiculopathy or associated stenosis.
The differential includes discogenic pain, sacroiliac pain, stenosis, spondylolisthesis, fracture, infection and visceral causes. Unusual night pain, fever, weight loss, trauma or neurological deficit changes the priority.

Observe positions and movements that change symptoms.

Distinguish local pain, referred pain and a radicular pattern.
Neurology
Document strength, sensation, reflexes, balance and dexterity as appropriate.

Clinical assessment and the role of imaging
Radiographs, CT and MRI may show osteoarthritis, hypertrophy, effusion or inflammatory changes, but asymptomatic abnormalities are common. Imaging is more useful when it answers a focused question such as stenosis, instability, fracture, tumour, infection or nerve compression.
The goal is to distinguish a plausible facet hypothesis from discogenic, sacroiliac, muscular, inflammatory or radicular pain and then choose care that matches the functional impact.

Conservative options and specialist interventions
Initial care usually emphasizes education, tolerable activity, progressive exercise and modification of provoking movements. Manual therapy may be included within an active programme. In selected persistent cases, medial branch diagnostic blocks or radiofrequency denervation may be considered by an appropriately qualified team.
Improvement after an intervention does not always prove that a facet joint was the only source. Decisions must remain revisable, especially when several degenerative findings coexist.

It is not the logical treatment for isolated facet pain. It may only be discussed when a concurrent disc, foraminal or radicular component is present.
When is prompt assessment needed?
Progressive weakness, bladder or bowel disturbance, saddle numbness, fever, major trauma or unusual night pain warrants prompt assessment.
How long should a plan be tried?
Timing depends on severity and goals, but the plan should state what will be measured and when it will be reviewed.
