The foramen is the opening through which a nerve root exits the spine. Its size depends on the disc, facet joints, ligaments and alignment. Combined disc and facet degeneration may produce local pain, radiculopathy or both.

Facet joints lie behind the foramen. Hypertrophy, osteophytes, a cyst or vertebral slip can reduce the exit space. Loss of disc height also decreases the vertical dimension of the foramen and may increase facet loading.

How can the facet joints narrow the foramen?

Foraminal stenosis can cause pain, tingling, numbness or weakness in a nerve distribution. Some people are limited mainly by extension or standing, whereas others react more to sitting or loading. Clinical behaviour varies with the dominant mechanism.

Isolated facet pain, disc-related radiculopathy, central stenosis, peripheral neuropathy and hip pathology can produce similar symptoms. Neurological examination and symptom distribution are essential.
