Segmentation variant in which the last lumbar vertebra is partly or completely united with the sacrum.
Pathology
Mechanism and manifestations
It is a lumbosacral transitional vertebra and may be asymptomatic. An anomalous joint or adjacent loading may sometimes contribute to pain, but the variant does not prove causation.
Diagnosis and management
Numbering should be confirmed before injection or surgery. Treatment addresses an identified symptomatic source rather than sacralization alone. Manual care cannot separate a bony fusion.
Points to consider
A congenital anomaly may be detected early or incidentally in adulthood. Its effects depend on associated structures and evolution, not simply its name. Follow-up may involve several specialties, particularly with neurological changes or progressive deformity.
Role of TAGMED care
This condition first requires medical diagnosis and, depending on the situation, specialist management. TAGMED services do not replace treatment of infection or tumor, stabilization of an unstable injury, or neurological or vascular treatment. Any conservative support should be considered only after diagnosis, restrictions and the treating team’s recommendations have been clarified.
Related concepts and distinctions
These concepts belong to the same field, but are not synonyms.
- Congenital block vertebra — Fusion of two or more vertebrae arising during embryonic development, distinct from surgical or acquired fusion.
- Lumbarization of S1 — Segmentation variant in which the first sacral segment retains lumbar-like characteristics and mobility.
- Hemivertebra — Congenital malformation in which part of a vertebral body does not develop normally, producing a wedge-shaped vertebra.
Conceptual guide: Sacralization of L5. Blocks summarize useful relationships; they are not an individual anatomical image.
