Experimental paradigm assessing how a conditioning painful stimulus changes perception of another painful stimulus applied at a distant site.
Physiology
Mechanism and function
Reduced test pain is interpreted as an inhibitory response within that protocol. Parameters, expectations and participant characteristics influence the result.
Assessment and clinical implications
This is not a stand-alone diagnosis of chronic pain. A weak response does not automatically determine treatment choice or outcome.
Points to consider
Pain is a personal experience influenced by biological and contextual processes. Modulation can increase or reduce a response without representing a new injury. Describing it neither questions the reality of pain nor establishes an individual diagnosis.
Role of TAGMED care
At TAGMED, assessment aims to understand symptoms, history and functional limitations. Manual care and osteopathy, motorized spinal decompression, laser and shockwave have different indications. The term described in this entry is not enough to select any of these modalities. Their relevance, limitations and possible contraindications should be discussed in relation to diagnosis and individual goals.
Related concepts and distinctions
These concepts belong to the same field, but are not synonyms.
- Temporal summation of pain — Increase in perceived pain during repeated similar noxious stimuli, used to investigate facilitation of pain processing.
- Gate control theory — Historical model proposing that spinal circuits modulate nociceptive transmission according to sensory inputs and descending influences.
- Descending pain modulation — Influence of brain and brainstem circuits on spinal processing of nociceptive information, which can inhibit or facilitate it.
Conceptual guide: Conditioned pain modulation. Blocks summarize useful relationships; they are not an individual anatomical image.
